Lab Requisitions

The Medical Services Commission (MSC) requires all laboratory diagnostic facilities in BC to use, and only accept, the Standard Out-Patient Laboratory Requisition (SOPLR) for out-patient laboratory test orders. The requisition undergoes periodic review and updating to include evolving laboratory protocols and ordering rules.

Certain fields must be completed to meet minimum requirements of the MSC to ensure positive patient identification and physician authorization. On BC Biomedical’s requisitions, these fields have been highlighted in yellow to guide you.

Note: It is important that requisitions be legible. Time taken by laboratory staff to verify a test request interrupts your office and increases wait times for our patients.
